This is a form is for ordering from instagram. You can write all the info and describe the dog tag that we have agreed on over Instagram.
Every tag will come with a split ring with the color of your choice.
The text style choice is only for the name, and the text color will be the same for both front and back, sometimes the number will be in a different color if it doesn't clearly visible (dog name and number).
